What is a Medicare wellness visit?

A Medicare wellness visit is an annual exam with a primary care provider for those who have Medicare. The purpose of this visit is to create or update a personalized plan to help prevent illness based on risk factors and the current state of health. It is not the same as the one-time Welcome to Medicare visit.

Who is eligible for Medicare wellness visits?

You are eligible for an annual Medicare wellness visit if:

You have had Medicare Part B for more than 12 months;
You did not have a Medicare wellness visit within the past 12 months; and
It is not within the same year as your Welcome to Medicare visit.

What should you expect in a Medicare wellness visit?

In your annual Medicare wellness visit, your primary care provider will tailor a prevention plan to suit your personal needs. You can expect:

Measurement of your weight, height, blood pressure, and other vital signs
Health risk assessment, which may include a questionnaire regarding your health status and needs
Review of your medical and family history
Review of your functional ability and health risks. This step may include screening for hearing loss and assessing your risk of falling and ability to perform daily activities.
Recording of your current medications, durable medical equipment, and healthcare providers
Written five to 10-year screening schedule
Screening for cognitive impairment, including Alzheimer’s and dementia, and for depression
Health advice or medical referrals to promote wellness and reduce identified risk factors
